The Lab Work Worth Doing Before Peptide Therapy
No blood tests at all is a red flag. A 60-marker panel is not automatically better. Here is the lab work that could genuinely change a peptide decision, and what should be monitored afterward.
A clinic that will start you on a peptide without a single blood test is telling you something about how it practices. But the opposite pitch, a 60-marker "optimization panel" before anything happens, is not automatically better. The right lab work is the set of tests that could change the decision, and it depends on what is being considered and who you are.
The short version
- Baseline labs exist to answer three questions: is there a reason not to start, is there another explanation for your symptoms, and what will you compare against later?
- A sensible core for most adults includes a complete blood count, a comprehensive metabolic panel (kidney, liver, electrolytes, glucose), HbA1c and a lipid panel.
- Peptides that act on the growth hormone axis make IGF-1 and blood sugar testing especially relevant. The label for tesamorelin, the approved GHRH analogue, specifically calls for IGF-1 monitoring and warns about glucose.
- Other tests, such as thyroid function, testosterone, pregnancy testing or cancer screening status, depend on your symptoms, sex, age and the medication in question.
- More tests are not always better. Screening healthy people for everything produces false alarms, costs money and rarely changes care.

What baseline labs are actually for
It helps to be clear about the job lab work is doing, because it is not "proving you need treatment".
- Safety screening. Some results make a medication a bad idea, or mean it needs closer monitoring: poorly controlled diabetes, kidney impairment, liver abnormalities, anemia, or a pregnancy.
- Finding the real cause. Fatigue, weight gain, poor recovery and low libido are the symptoms that bring people to peptide clinics. They are also classic presentations of thyroid disease, iron deficiency, poorly controlled blood sugar, sleep apnea, depression and low testosterone. Treating the symptom without looking for the cause is how things get missed.
- A baseline for comparison. If a medication might raise blood sugar or IGF-1, you need a starting value so that a later result means something.
The core panel most adults should expect
For most adults being assessed for any injectable prescription, including peptides, a reasonable starting point is:

Alongside the labs, a clinician should record blood pressure, weight and a full medication and supplement list. Those are not blood tests, but they are just as much part of the baseline.
Tests that depend on what is being considered
Growth hormone axis peptides
Tesamorelin, and unapproved secretagogues such as CJC-1295, ipamorelin and sermorelin-type products, work by increasing growth hormone and, downstream, IGF-1. The tesamorelin prescribing information is the best available guide to what this class can do, because it is the one with FDA-reviewed data. It advises monitoring IGF-1 during treatment, warns that the drug can worsen glucose tolerance and that new diabetes developed more often on treatment than placebo, and lists active malignancy and pregnancy as contraindications.
So before any growth-hormone-axis peptide, a careful workup typically includes:
- IGF-1, both to understand your starting point and to monitor later.
- HbA1c and fasting glucose, with a plan to recheck.
- A review of cancer history and whether age-appropriate screening is up to date, since growth hormone and IGF-1 can influence cell growth.
- Pregnancy testing where relevant.

GLP-1 and related weight-management medicines
Semaglutide and tirzepatide are FDA-approved peptide medicines, and their labels shape the screening. Their boxed warning concerns thyroid C-cell tumors, so a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia type 2 is a contraindication; that is established by history rather than a routine blood test. Kidney function matters because dehydration from nausea or vomiting can affect the kidneys, and HbA1c matters for anyone who may have undiagnosed diabetes or who takes other glucose-lowering drugs. Bremelanotide (PT-141)
For bremelanotide, the key baseline is cardiovascular rather than a blood panel. The Vyleesi label lists uncontrolled hypertension and known cardiovascular disease as contraindications and describes a transient rise in blood pressure after each dose. Tests driven by your symptoms
Many people arrive asking for a peptide when what they want is to feel less tired, recover better or regain libido. Depending on the story, a clinician may reasonably add:
- Thyroid function (TSH, sometimes free T4) for fatigue, weight change, feeling cold or hair thinning.
- Iron studies and ferritin for fatigue, heavy periods, restless legs or a low hemoglobin.
- Testosterone in men with symptoms of deficiency. Guidance from the Endocrine Society calls for morning, fasting measurements and a repeat test to confirm a low result, because levels vary. See testosterone blood testing.

Worth knowing
Not every popular test earns its place. The Endocrine Society's 2024 guideline on vitamin D for disease prevention suggests against routine vitamin D blood testing in the general healthy population, because there is no good evidence that screening and treating to a target number improves outcomes. A clinician may still check it when there is a specific reason. The principle generalizes: a good test is one whose result would change what you do.
Why a giant panel is not the gold standard
Some clinics market extensive panels as proof of thoroughness. There are real downsides:
- False positives. Reference ranges are usually set so that about 5% of healthy people fall outside them on any single test. Order enough tests and an "abnormal" result becomes likely even in a healthy person.
- Cascades. An incidental finding can lead to repeat tests, imaging, anxiety and cost, often ending where it started.
- False reassurance. A long list of normal results can make a service look rigorous while the questions that matter, such as your cancer history, medications or sleep, go unasked.
- Selling by numbers. "Suboptimal" ranges that differ from standard laboratory references can be used to turn a normal result into a reason to buy something.
Prostate cancer screening is a useful example of a test that needs a conversation rather than a reflex. The US Preventive Services Task Force's 2018 recommendation is that for men aged 55 to 69, PSA screening should be an individual decision after discussing potential benefits and harms. Folding a PSA into a panel without that discussion skips the part that matters.
Be careful here
Watch for a service where the lab results always seem to justify the product. If every patient's IGF-1 is "suboptimal" and every testosterone level "could be higher", the labs are marketing, not medicine. Ask what result would lead them to advise against treatment. If they cannot name one, that is your answer.
Monitoring after you start
Baseline testing is only half the job. Before you start, a clinician should tell you:
- Which tests will be repeated, and roughly when.
- Who reviews the results, and how quickly you will hear.
- Which findings would lead to a dose change, a pause or stopping altogether.
- What symptoms should prompt you to get in touch between scheduled tests.
If you are on an unapproved peptide, there is no FDA-reviewed label telling anyone what to monitor, which makes a thoughtful plan more important, not less.
Practical tips for your blood draw
- Ask whether to fast. Fasting affects glucose and triglycerides, and some hormone tests are timed for the morning.
- Mention biotin supplements. High-dose biotin, common in hair and nail products, can interfere with some laboratory tests, including certain thyroid and hormone assays. Ask whether to pause it beforehand.
- Keep conditions similar for repeat tests. Same time of day, similar fasting status and, ideally, the same lab make results easier to compare.
- Bring previous results. Recent labs from your primary care clinician may mean fewer new tests, and trends over time are more informative than a single value.

What blood work is needed before peptide therapy?
It depends on the peptide and on you. A common core is a CBC, comprehensive metabolic panel, HbA1c and lipids, with IGF-1 added for growth hormone axis peptides and other tests guided by symptoms, sex and age.
Do I need to check IGF-1 before sermorelin, CJC-1295 or ipamorelin?
These peptides are intended to raise growth hormone and IGF-1, so a baseline IGF-1 and a plan to recheck it are reasonable. The approved relative, tesamorelin, carries a label instruction to monitor IGF-1.
Can I use lab results from my regular doctor?
Often yes, if they are recent and cover what is needed. Share them before your consultation so you are not paying to repeat tests.
Is an at-home finger-prick test good enough?
Some are reliable for specific markers, but results can vary with collection technique and not every test is validated on capillary blood. Ask your clinician whether a particular kit is acceptable for the decision being made.
Why won't a clinician just order every test I ask for?
Because tests without a clear purpose can create false alarms and unnecessary follow-up. A clinician who explains why a test is or is not useful is practicing good medicine.
