MOTS-c: The Mitochondrial Peptide Everyone Is Talking About

A peptide hidden in your mitochondrial DNA, released by exercise, that made old mice run further. It is a great story. Here is how much of it has actually been shown in people, and where the law stands.

MOTS-c has the kind of backstory that travels well: a peptide hidden in your mitochondrial DNA, released when you exercise, that made old mice run further. Every part of that sentence is based on real research. What it leaves out is that almost all of it was done in mice, and that no MOTS-c product has been shown to do any of this for people.

The short version

  • MOTS-c is a 16-amino-acid peptide encoded in mitochondrial DNA, first described in mice in a 2015 Cell Metabolism paper.
  • In mice, it acted on muscle metabolism and protected against diet-induced weight gain and insulin resistance. In a 2021 study, treating old mice improved measures of physical capacity.
  • In people, exercise raises MOTS-c levels. That is not the same as showing that injecting it helps.
  • The only human drug data we found are for CB4211, a modified analogue, in a four-week early-phase study reported by the company in 2021. There are no published trials showing that MOTS-c injections cause weight loss or slow aging.
  • An FDA advisory committee narrowly recommended MOTS-c for the compounding bulks list in July 2026 (7-5, 2 abstentions), against FDA reviewers' view. That vote is not binding. MOTS-c is not FDA-approved.
  • MOTS-c is named on the WADA Prohibited List and banned in and out of competition.

MOTS-c has gone from an obscure research term to something clinics sell as a weight-loss, energy and longevity injection within a few years. If you have been offered it, or you are trying to decide whether the hype is worth a consultation, here is what the research does and doesn't support, and where the law stood as of September 2026.

What MOTS-c is, and why "mitochondrial" matters

Almost all of your DNA sits in the nucleus of your cells. A small circle of it sits inside the mitochondria, the structures that turn food and oxygen into usable energy. For a long time that mitochondrial DNA was thought to code for only a handful of proteins involved in energy production.

Over the past two decades, researchers found short stretches within it that can produce small peptides. These have been called mitochondrial-derived peptides. MOTS-c is one of them; humanin is another. The name stands for "mitochondrial open reading frame of the 12S rRNA-c", which describes where in the mitochondrial genome its code sits.

The idea that mitochondria might send out their own chemical signals to the rest of the body is a genuinely interesting one in biology. It is also a long way from a treatment.

What the mouse studies found

Lee and colleagues, 2015

The paper that introduced MOTS-c, by Changhan Lee and colleagues in Cell Metabolism in 2015, reported that in cells and in mice, MOTS-c acted mainly on skeletal muscle. It affected a metabolic pathway that led to activation of AMPK, an enzyme that acts as a cellular energy sensor. In mice, MOTS-c treatment prevented the insulin resistance that came with a high-fat diet and with age, as well as diet-induced obesity.

A later study by Reynolds and colleagues in Nature Communications had two parts. In ten young, sedentary men, a session on a stationary bike raised MOTS-c levels in muscle substantially and in blood more modestly. In mice, treatment started late in life, three times a week, improved grip strength, gait and physical performance compared with untreated old mice.

That second finding produced headlines about an "exercise protein" that doubled running capacity in mice. It is striking work. It is still mouse work.

Worth knowing

A molecule that rises during exercise is not necessarily one that delivers exercise's benefits when injected. Exercise changes hundreds of molecules at once, alongside blood flow, muscle loading, heart rate and hormones. Showing that one of them matters on its own, at a particular dose, in humans, requires controlled human trials. For MOTS-c, those haven't been published.

What we know in humans

This is the short section, which is the point.

  • Observational data. Studies have measured natural MOTS-c levels in people and compared them across ages and health conditions, and the Reynolds study showed levels rise with exercise. Associations like these can't tell you whether taking MOTS-c would change anything.
  • CB4211, an analogue. The company CohBar developed a modified version of MOTS-c called CB4211. In 2021 it announced topline results from a phase 1a/1b study: 65 healthy adults in the first part, then 20 adults with obesity and fatty liver disease randomized to daily injections or placebo for four weeks. The company reported reductions in liver enzymes and blood glucose compared with placebo, a trend toward lower body weight, and reductions in liver fat in both the treatment and placebo groups. Injection site reactions occurred in more than 10% of people on the drug.

Those results came from a press release on a small, short study of a different molecule. They aren't nothing, but they don't support the specific claims made for MOTS-c injections, and we are not aware of later-phase results being published.

Be careful here

If you are told MOTS-c is "clinically proven" for fat loss, "reverses metabolic aging" or is "your body's own peptide, so it's safe", each claim goes beyond the evidence. There is no established human dose, no long-term safety data, and no information about interactions with diabetes medications, which matters if a substance affects blood sugar. Being naturally present in the body does not make an injected, manufactured version harmless.

The regulatory picture in 2026

MOTS-c has been at the center of this year's peptide policy changes. Here is the sequence, as of mid-September 2026.

  1. February 27, 2026. HHS Secretary Robert F. Kennedy Jr. announced a plan to revisit FDA restrictions on a group of peptides.
  2. April 2026. FDA removed MOTS-c and several other peptides from "Category 2", its list of bulk substances raising significant safety risks, so they could be reviewed. That removal did not make them eligible for compounding: they were never in Category 1 or on the 503A bulks list.
  3. July 2026. FDA's Pharmacy Compounding Advisory Committee voted 7-5, with 2 abstentions, to recommend adding MOTS-c to the 503A bulks list. FDA's own reviewers had concluded that none of the seven peptides considered at the meeting met the criteria.
  4. Now. Committee votes are advisory. To add a substance to the bulks list, FDA must go through notice-and-comment rulemaking. As of August 2026, legal analyses indicated these peptides still could not lawfully be compounded from bulk, and no final rule had been reported as of this writing.

"Research use only" MOTS-c

Much of the MOTS-c in circulation is sold as a research chemical, "not for human consumption". Those products are outside the pharmacy system altogether, with no verified identity, purity or sterility. The risks are set out in the research peptide grey market.

Athletes: MOTS-c is explicitly banned

This one is unambiguous. The World Anti-Doping Agency added MOTS-c by name to its Prohibited List in 2024, under S4, Hormone and Metabolic Modulators, as an example of an AMPK activator. It remains named on the 2026 list and is prohibited at all times, in and out of competition. USADA has noted that it is heavily marketed by wellness and anti-aging clinics despite being an experimental peptide. A prescription does not change that. If you are tested under WADA rules, a college program or a professional league, read peptides and drug testing before taking anything.

What MOTS-c is being sold for, and better-supported routes

MOTS-c tends to be offered for three goals. Each has approaches with far stronger human evidence.

Weight loss and blood sugar

FDA-approved GLP-1 medicines such as semaglutide and tirzepatide have large, long trials behind them for weight management in eligible adults, along with known side effects and monitoring requirements. They aren't right for everyone. But if weight or blood sugar is the goal, they are the evidence-based conversation to have. Energy and exercise capacity

The irony of MOTS-c marketing is that its best-documented human finding is that exercise raises it. Fatigue also has many treatable causes, including thyroid problems, anemia, poor sleep, sleep apnea and medication effects, which the right lab work can help identify.

Talking to a clinician about MOTS-c

Common questions

Is MOTS-c FDA-approved?

No. MOTS-c is not FDA-approved for any use. An advisory committee recommendation in July 2026 does not change that, and it is not a decision to allow compounding.

Does MOTS-c help with weight loss?

In mice, MOTS-c prevented diet-induced weight gain and insulin resistance. There are no published controlled trials showing MOTS-c injections cause weight loss in people. A related analogue, CB4211, showed a trend toward lower body weight in a small four-week study reported by its developer.

Is MOTS-c banned in sports?

Yes. It is named on the WADA Prohibited List under S4 as an AMPK activator and is prohibited at all times. Check with USADA or your sport's anti-doping authority for your specific situation.

Can I legally get MOTS-c from a compounding pharmacy?

As of August 2026, legal analyses indicated it still could not lawfully be compounded from bulk under section 503A, despite the committee vote, because FDA had not completed rulemaking. This may change, so verify the current status with FDA or a clinician.

Does exercise increase MOTS-c naturally?

In a 2021 study of ten young sedentary men, a single exercise session raised MOTS-c levels in muscle and, to a lesser degree, in blood. Whether that rise explains any of exercise's benefits in people hasn't been established.
